Aurora Concepts will come to the school to administer the required immunizations needed for the students’ entering into 7th grade, 11th grade, and 12th grade.

If your child is entering into the 7th grade (1) dose of Meningococcal (MCV4) and (1) dose of Tdap is required.

Aurora Concepts will provide the second dose of MCV4 (Meningococcal) immunization to the 11th graders who will be taking college classes and to the 12th graders who need the second dose of MCV4 (Meningococcal) that are graduating before entering into college.

Parents’ if you are interested in having your student immunized please complete and sign a few papers that are required. Parents’ this helps you from taking off work and one less thing to worry about over the summer. The forms must be completed and signed along with copy of the front and back of the students’ insurance card, as stated below in order to get their shots. If no insurance, each vaccine required will be $5 each and the money will need to be sent to the Nurse’s Office along with the forms completed.

Immunization Clinic is scheduled for May 9, 2018.

Each student will need:

v Signed Vaccine Consent - Vaccine Consent Form

v Signed ImmTrac Consent - Immunization Registry Minor Consent Form

v Copy of your Insurance Card - Front & Back

v Return completed forms back to Nurse’s Office by April 16, 2018

Again, also note, that if you do not have any insurance, the cost for immunization required would be $5 each immunization.

Please check your child’s immunization record in 6th grade to make sure they are not delinquent; and to make sure the nurse has a recent copy of immunizations.

If you have any questions, please feel free to contact me in the office 636-7832 Ext. 237.

 Sincerely,

Lovelady ISD School Nurse
