Faculty may go to our website:   www.auroraconcepts.net    and click on the school login in the right upper corner.  Then click on your school, the guest password may be requested from your District Nurse. Next register and select the items you wish to receive.  The 'FORMS' link is on the website homepage.   Please print and fill out the faculty consent form to bring with you on the date of service.  Also, bring a front and back copy of your insurance card.  If they have any questions, I may be contacted at  or 936-598-3296(office) or 936-590-2906(cell - text is good). If someone does not have Aetna insurance they can fax or email a front/back copy of their insurance card for verification of coverage to .
